During this telephone call, you and Dr. Rosenthal will discuss your treatment concerns to determine if she is the right doctor to meet your mental health needs.

Psychiatric Evaluation - 75 minutes ($400)

During this appointment, Dr. Rosenthal will take a comprehensive psychiatric, medical, social, and family history in order to understand and address your concerns. Please have a list of current and former medications, dosages, effects, and potential side effects. We will work

together to develop a treatment plan which may include medication management, further workup such as bloodwork, and/or referral to a psychotherapist.

Psychiatric Follow Up - 30 minutes ($200)

Follow up appointments may occur in intervals ranging from one week to three months, determined by shared decision making. Follow up frequency varies depending on stability, acuity, diagnosis, treatment plan, and response to treatment. During these appointments, Dr. Rosenthal will assess treatment response and concerns, and may make changes to the treatment plan.

Disclaimer

1. No Show: No shows within 24 hours of scheduled appointment time will be charged the full fee.

2. Medication Refill Requested Outside of Scheduled Appointment: If you are taking a medication prescribed by Dr. Rosenthal, it is your responsibility to ask for and obtain a refill at the time of your appointment. Any refills requested outside of your regular appointment will be charged at a rate of $80.

3. Other Professional Services: This includes telephone calls to/from you, your pharmacy, your physician, or insurance company, and for the completion of forms or case-related correspondences. This will be charged at a rate of $100 per fifteen minutes.
